communication and coordination among our RN case managers and each participant,
his or her health care provider and family or caregiver, other vendors, and available
community resources.

reduces treatment costs, and helps participants live healthier, more productive
lives. We focus targeted interventions to help those living with chronic conditions
better manage their illnesses. By focusing on overall wellness and lifestyle management,
we expect to deliver an enhanced and more measurable ROI than traditional disease
management programs. We use behavior change principles, educational tools and information,
and RN case manager coaching support to get conditions under control.
Maternity management identifies and engages expectant
women early in their pregnancies to keep moms and babies healthy. RN case managers
work with mothers-to-be to help them change unhealthy behaviors and manage either
existing conditions or those that may arise during pregnancy. This reduces the risk
of complications, resulting in healthier babies and lower medical costs associated
with high risk pregnancies and neonatal intensive care.
